For the fastest, most convenient way to access our GP services.
At Southglade Medical Practice, we use an online appointment system for the booking of same-day appointments and will only accept telephone requests under exceptional circumstances. The system opens at 8am Monday to Friday and will close to new requests once we have reached full capacity. You can still call the Practice to arrange routine appointments for the following:
All appointment requests will be triaged by a GP. Our Reception/Admin team will then contact you to advise you of the outcome i.e. a GP/ANP appointment, other clinician appointment, referral to a local pharmacy or service, or over-the-counter/self-management.
When submitting a request form, please ensure you select the most appropriate form for your query. Please be advised that we do not process requests for medical problems if they have been submitted via the ‘Admin query’ form; these will be rejected and you will be asked to complete the most relevant form available – therefore, this will delay your request for an appointment.
Note: you may be asked if you have a preference for the type of appointment (face-to-face, telephone, virtual or email). Whilst we always try to accommodate patient preference, the triaging GP will note the most appropriate appointment type required for the request.

If you’re unable to download the app or don’t want to use online services, please contact the practice directly by phone to make a request.
We provide a wide range of services. There are a range of professionals who can look after you. You may be offered an appointment with an advanced nurse practitioner, physiotherapist, paramedic or clinical pharmacist rather than a GP.
This appointment may take place face-to-face at the practice, on the telephone or via video call.
Before you make an appointment to see your GP, think about what other services might be able to help.
You do not have to make an appointment or register for these services.
We understand that sometimes you may not be able to attend your booked appointment. If you cannot attend, help us by letting us know as far in advance that you cannot make your appointment. This means we can offer the appointment to another patient who needs it.

Pharmacists can now provide treatment or some prescription medicine, if needed, for seven common conditions, without having to see a GP.

If you cannot use Dr.iQ, please call the practice and one of our receptionists will complete this for you. You can find our contact details, here.
You will receive a response the same day confirming how we will mange your request.
If you can’t make your appointment, or it’s no longer required, please cancel it as soon as possible so we can offer it to another patient. We are able to visit very poorly and housebound patients in their homes. If you need a home visit please call the surgery by 10am so the Doctor can plan their rounds. Home visits are normally made between 12.30pm and 3.30pm.
You can view your medical record using the Dr. iQ powered by Evergreen Life app, or NHS app. The medical record feature gives you access to information such as: test results, medications, documents and letters, immunisation history, consultation history and allergy information.
